The tremor occurred at 10:59 p.m. (1459 GMT) on Tuesday near Lagayan town in Abra province, 348 kilometers north of Manila, according to the Philippine Institute of Volcanology and Seismology.
According to the institute, the earthquake had an initial magnitude of 6.7 but was later downgraded to 6.4.
Based on an initial assessment, power was cut in some affected provinces, while houses and buildings were damaged, and some roads were impassable due to rock slides or cracks, according to the national disaster agency.
Photos posted on Facebook by the Bureau of Fire Protection show a hospital in nearby Ilocos Norte province being evacuated after the ceiling collapsed.
